Procedure: Form Creation and Programming

I need an online form, application and/or survey. How do I get one? When will it be ready?

A basic form submits all information to an e-mail address. None of the information submitted in the form is stored in a database. Please allow two weeks for the completion of a basic form. To request a basic form, submit a Web Request Form.

Please provide the following:

  • Text that you want to appear at the top of the form to tell the user the purpose of the form or who to contact if they have questions.
  • Text fields to be included on the form: e-mail address, first name, last name, phone number.
  • Text fields that are required to be completed in order to successfully submit the form. For example, if someone is required to enter their phone number in order to submit the form, we need to know that.
  • Do you or others need to be notified of successful submissions with a copy of the completed form? If so, specify e-mail address(es)
  • Does the submitter need to be e-mailed a confirmation e-mail?
  • Thank you text to be displayed to the user once the form is completed
  • Indicate the e-mail address where the completed form should be submitted

More complex forms that store information in a database can be built based on time and resources available at the time of your request. Web Communications reserves the right to deny requests based on resources available.
